NV COTC | Premarital & Wedding Officiant Request Form

PLEASE READ CAREFULLY. (Both parties need to complete this form individually).

Congratulations on your upcoming wedding!

As a faith community, we long to help those approaching marriage lay a solid, Christ-centered foundation on which to build a life of mutual love and service. To help us best serve you, please answer the following questions. Once we receive your request, a member of our team will follow up with you.

To ensure there is enough time to sufficiently complete the premarital process, we require your wedding to be at least six months from the day you submit this request. If your wedding is less than six months away, please email care@churchofthecity.com to be provided with potential premarital counseling referrals.

To prepare couples for marriage, we believe it is important to help engaged couples examine their relationship by exploring areas related to family of origin, spiritual and relational history, their sexual relationship, as well as the mental, emotional, and spiritual health of both individuals. By submitting this request, you understand this arrangement and willingly invite your officiant and/or premarital mentors into this process.

The answers you provide here will be kept confidential to the pastoral team and your officiating pastor and/or mentor couple. We ask that each person complete this form individually and separately. Please be honest and thorough in your responses to help us serve you best.
Date

Due to the high volume of requests received, we are only accepting couples who are actively involved at Church of the City.

WEDDING INFORMATION

We ask that couples requesting a COTC pastor to officiate their wedding engage in premarital counseling through COTC or another appropriate provider. If you have a different plan for premarital counseling, you can discuss that with a potential officiant when you meet.

Given the size of our church and the number of requests we receive, we are only able to offer premarital mentoring and provide officiants for couples who are Stakeholders or regular attenders at COTC.

If you are a Church of the City Stakeholder, one of our pastors may be able to officiate your wedding. While we desire to find an officiant for those who call COTC home, we cannot guarantee a pastor will be available on your wedding date. However, considering personal schedules and other ministry commitments, we commit to doing all we can to have a COTC pastor officiate your special day.
